Questions & Answers

Who is included in the Valley Oaks settlement?

The class covers people whose private information was potentially compromised in Valley Oaks Health's June 2023 data incident and who appear on the settlement class list. The potentially exposed information varied by person and may include identity, medical, financial or account-access information.

The assigned judge, court personnel and their immediate families are excluded. Valley Oaks and specified related parties are excluded, as is anyone found guilty of causing or helping cause the incident or who pleads no contest to such a charge.

Which benefits can I combine?

Every class member may claim two years of medical monitoring and choose between two cash paths. One path permits qualifying ordinary-loss, extraordinary-loss and lost-time claims. The alternative path provides $40 instead of any other cash payment.

  • Up to $500 for ordinary documented losses.
  • Up to $5,000 for extraordinary identity-theft or fraud losses.
  • Up to $80 for four hours of lost time at $20 per hour.
  • Or a $40 alternative cash payment instead of the other payments.

What losses may qualify for reimbursement?

Ordinary losses may include credit-report or monitoring charges, credit-freeze costs, replacement identification and postage used to contact banks. Extraordinary reimbursement covers identity-theft or fraud losses probably caused by the incident if they are not also claimed as ordinary losses and you made reasonable efforts to prevent or recover them.

Losses must have occurred between June 8, 2023 and November 9, 2026 and cannot already have been reimbursed by someone else.

What proof is required for a payment?

Receipts or similar third-party records are required for ordinary and extraordinary loss reimbursement. Self-prepared notes can explain other evidence but cannot establish those claims by themselves. Lost-time claims require the hours and a brief description of the work.

The $40 alternative cash payment does not require proof or an explanation. Every benefit still requires a valid claim.

How do I submit a claim, and when is it due?

Submit the official claim online by November 9, 2026, or mail a completed and signed form postmarked by that date to the settlement administrator. Include supporting records if requesting ordinary or extraordinary loss reimbursement.

The official settlement website contains the online form and a printable claim form. Keep a copy of the submission and supporting documents.

When will settlement benefits be issued?

The Court is scheduled to consider final approval on December 10, 2026 at 2:00 p.m. Eastern Time. Payments and services will be distributed only after final approval and resolution of any appeals. The notice does not provide a distribution date; check the official site for hearing updates.

What happens if I do nothing?

You will receive no payment or monitoring service. Unless you validly opt out, you will remain in the settlement and give up the right to pursue the released claims against Valley Oaks separately.

How is opting out different from objecting?

Opting out removes you from the settlement. You receive no settlement benefit but retain any individual rights concerning the released claims. Objecting asks the Court not to approve some or all of the settlement while you remain a class member. You cannot object after opting out.

  • Opt-out requests must be submitted, postmarked or emailed by November 9, 2026 and must follow every requirement in notice question 15.
  • Objections must be filed with the court and copied to the administrator by November 9, 2026; notice question 16 lists the required information.
