What losses may qualify for reimbursement?
The up-to-$4,000 option is for actual, unreimbursed out-of-pocket losses caused by the Data Incident. Costs for monitoring or identity protection must have been first purchased between July 24, 2025 and November 30, 2026, primarily because of the incident.
- Unreimbursed bank or credit-card charges.
- Costs to replace a driver's license, Social Security number or other identification.
- Costs for credit reports.
- Qualifying credit-monitoring or identity-theft-protection purchases.
What proof is required for a payment?
Third-party records, such as receipts, are required for documented-loss reimbursement. Notes you prepare yourself may explain other proof but cannot support that option by themselves.
The $55 alternative cash payment does not require third-party documents. Lost-time reimbursement requires the number of hours and a description on the claim form. Every option still requires a truthful, signed claim that the administrator may verify.
How do I submit a claim, and when is it due?
Submit the official claim online by November 30, 2026, or mail a completed and signed form postmarked by that date. Include supporting documents if you request documented-loss reimbursement.

When will benefits be distributed?
The Court is scheduled to consider final approval on December 15, 2026 at 9:00 a.m. Eastern Time. Benefits will be distributed only if the Court grants final approval and any appeals are resolved. The notice does not give a payment date. Check the official site before the hearing because the date or location may change.
What happens if I do nothing?
You will not receive monitoring or a cash benefit. Unless you validly opt out, you will remain in the settlement and give up the right to pursue the released claims against Jack's on your own.
How is opting out different from objecting?
Opting out means leaving the settlement. You receive no settlement benefit but keep any individual rights concerning the released claims. Objecting means asking the Court not to approve some or all of the settlement while remaining a class member. You cannot object after opting out.
- Opt-out requests must be mailed and postmarked by November 13, 2026. Follow notice question 15 for every required item and the mailing address.
- Written objections are due to the administrator by November 13, 2026. Notice question 16 contains a detailed ten-part checklist, including prior objection history, lawyer and witness information, hearing plans, any use of artificial intelligence, and your personal signature.
