How do I know whether my information was involved?
The class covers individuals whose private information was accessed, acquired, disclosed or compromised in the April 23-May 5, 2024 incident, subject to the listed exclusions. A mailed settlement notice identifies you as eligible to submit a claim.
If unsure, contact the administrator at 1-844-440-4203 or [email protected]. The incident dates describe the cybersecurity event; the notice does not establish a purchase period or a separate fixed loss-date window.
Can I receive both cash options and credit monitoring?
Choose one cash option: Cash Payment A for documented losses, stated up to $5,000, or Cash Payment B for an estimated $60. The two cash options cannot be combined.
You can add two years of single-bureau credit monitoring to either cash choice, or request monitoring alone. Select the benefits you want on the claim form; they are not sent automatically just because you received a notice.
Is the $60 payment guaranteed?
No. It is an estimate. Notice Question 9 says cash payments can increase or decrease proportionally depending on valid claims and the net fund. The administrator determines final amounts after reviewing the claims.
The $3.1 million fund also pays administration and approved legal fees, costs and representative awards. Counsel plans to request fees up to one-third of the fund plus reasonable costs, and $2,500 for each of five representatives. The court may award less.
What evidence supports a documented-loss claim?
Choose Cash Payment A and list actual, unreimbursed expenses related to the incident. Include clear copies of supporting records, such as receipts, telephone records or correspondence, and check the separate loss attestation on the form.
Contemporaneous records prepared by you or a third party may qualify. A personal certification, declaration or affidavit alone does not; it can explain other records. Expenses already repaid through another source, including prior identity-protection coverage, cannot be recovered again. Keep originals and a copy of the claim because submitted materials are not returned.
How do I activate the credit monitoring?
Check the monitoring box and provide an email address. Form Section IV says the activation code will be emailed after final approval and after the settlement becomes effective. Follow the activation instructions when it arrives.
The service covers one credit bureau for two years and includes dark-web monitoring and $1 million in identity-fraud insurance. That insurance limit is not an additional cash payment. What if I do not have a Notice ID or Confirmation Code?
The online form requires both codes from your personalized notice. The paper form asks for a Notice ID only if known. You can download that form or call the administrator to request a copy and help with membership or missing codes.

Which address should I use to opt out?
Opt-outs go to a different post office box from claims: Palomar Health Data Incident Settlement, Attn: Exclusions, P.O. Box 59, East Brunswick, NJ 08816-9998. Mail your personally signed letter postmarked by October 7, 2026.
Include your name, address, phone, email if any, and a clear request to leave the class. Requests must be individual; mass opt-outs are not allowed. Leaving means no benefits but preserves any rights you have to pursue the released issues separately.
How do I object while remaining eligible to claim?
Submit an objection by October 7, 2026 to the Clerk of Court and send the required copies to class counsel, defense counsel and the administrator. Notice Question 22 and page 7 give all addresses, mailing rules and required details, including your own signature, reasons, prior objections, attorney information and hearing plans.

When is the hearing, and what happens if I do nothing?
The notice schedules the hearing for November 6, 2026 at 10:30 a.m. in Courtroom SD-64, Hall of Justice, 330 W. Broadway, Fourth Floor, San Diego. It does not state a time zone. Attendance is optional; follow Question 22 if asking to speak and check the official site for changes.
There is no definite payment date because approval, any appeals and claim processing must finish. Doing nothing gives you no benefits. Unless you opt out, a final settlement binds you to the release of claims described in the agreement.
