Does having been a Regional Urology patient automatically qualify me?
No. The class is living individuals in the United States whose private information was potentially compromised in the October 2025 incident, subject to the listed exclusions. A patient relationship alone does not establish membership.
The affected files may have included identifiers and information about care before December 31, 2022. That care date describes the records involved; it is not a purchase window. If unsure about membership, contact the administrator at 833-200-7004 or [email protected].
Can I get the $40 payment and medical monitoring?
No. The $40 alternative replaces both medical monitoring and reimbursement for documented losses. If you choose the other option, you may select one year of monitoring, up to $5,000 in eligible losses, or both.
The monitoring includes medical identifier and HSA-spending alerts, fraud-resolution support and up to $1 million of medical identity theft insurance. That insurance limit is not money paid to every claimant.
Which losses can I claim, and what records do I need?
The notice covers actual expenses caused by the incident, such as fraud losses, credit monitoring or freeze fees, replacement IDs and postage to banks. You cannot recover expenses already reimbursed by another party. Attach records showing the amount and connection to the incident; a personal explanation alone is insufficient.
The notice and form overview give October 5, 2025 through November 16, 2026 as the loss window. Form Section III more specifically asks for losses after October 5 and up to the date you file. Claim losses already incurred; ask the administrator about an October 5 loss or a later supplement rather than assuming it is accepted. The form allows additional signed expense sheets.

What if I cannot find my Login ID or PIN?
The online portal requires both. Its instructions say to request them by emailing the administrator with your full name and mailing address. The paper form asks for a Login ID only if known, so there is also a paper route.

Can I email my claim, and how do I choose payment?
Yes. Page 3 of the official claim form explicitly allows an electronic image of the completed form to be emailed to [email protected]. Online filing and U.S. mail are also available. Submit by November 16, 2026; mailed claims need a postmark by that date.
Complete the benefit selections and attestation, attach any loss records and choose PayPal, Venmo, Zelle, virtual prepaid card or a physical check. Keep your contact information current and respond if the administrator requests more information. Do not send your claim to the court or lawyers.
Why does the claim form ask about Medicare?
The last page asks whether you were a Medicare beneficiary from October 5, 2025 to the present and are seeking reimbursement for emotional distress. If both apply, it directs you to call the administrator for Medicare reporting information. Otherwise it says to leave that box unchecked.
That reporting question does not itself promise an additional emotional-distress payment or change the stated benefit limits. Ask the administrator how it applies to your claim.
How do I leave the settlement?
Send a personally signed request with the case name and number, your full contact details and a clear statement that you want exclusion. Notice Question 15 says it must be submitted, postmarked or emailed by October 16, 2026. The mail address is OLHS Data Incident Settlement, ATTN: Exclusion Request, P.O. Box 25226, Santa Ana, CA 92799-9958; the administrator contact email is [email protected].
You may exclude only yourself. Leaving means no settlement benefits, while keeping any rights you have to bring the covered issues separately.
How do I object without leaving the class?
File a complete objection with the Clerk of Court, 501 Texas Street, Room 103, Shreveport, LA 71101 by October 16, 2026. Also send copies to the administrator, class counsel and defense counsel at the addresses on notice page 8.
Question 16 lists the required membership proof, reasons, legal support, lawyer and prior-objection details, proposed witnesses/documents, hearing plans and your personal signature. An objection is separate from a claim; you can still request benefits if you stay in the class and file a valid claim.
Why is the hearing before the claim deadline, and when will payments arrive?
The notice and official site schedule the hearing for October 19, 2026 at 9:30 a.m. Central, at 501 Texas Street, Suite 801, Shreveport, Louisiana. The claim deadline remains November 16, 2026. These are separate dates; attendance is optional and the hearing can change.
Payments require approval and resolution of any appeals, so there is no definite payment date. OLHS will separately pay approved legal fees and costs requested at $265,000 and requested representative awards of $3,000 each; the notice does not state a total settlement fund.
