LSC Data Incident Settlement

The formal class definition is based on possible compromise of personal information in the data incident. It is not limited in the notice to people who received an individual notice.

• You must be a U.S. resident whose personal information was potentially compromised in the LSC data incident.
• You are excluded if you are the presiding judge or an immediate family member, LSC or certain related entities and their current or former officers and directors, or if you timely opt out.

The notice says class members may claim all three types of benefits. Each requires a timely, valid Claim Form. The loss reimbursement has additional documentation requirements.

• Up to $5,000 for qualifying unreimbursed out-of-pocket losses caused by the incident.
• A pro rata cash payment of up to $1,000.
• Two years of Medical Shield Complete by CyEx.

The maximum reimbursement is $5,000 per person. The loss must have been reasonably incurred because of the data incident and must not already have been reimbursed.

• Fraud or identity-theft losses that were not reimbursed.
• Professional fees, such as attorney, accountant, or credit-repair fees.
• Costs related to freezing or unfreezing credit, qualifying credit-monitoring costs, and certain expenses such as notary, fax, postage, copying, and mileage.

A signed claim form, declaration, or notice identification by itself is not proof of a loss. Self-prepared documents may help explain other evidence, but are insufficient on their own for reimbursement.

• Provide receipts or other records showing the costs you incurred.
• Do not rely only on records you created yourself, such as handwritten receipts.
• The notice specifically requires supporting documents for an out-of-pocket-loss claim. It does not clearly say whether separate documents are required for a cash-payment or monitoring claim.

Because the notice does not explain the Settlement Subclass Payments, it does not provide enough information to calculate an expected cash payment.

• The cash payment is not guaranteed to be $1,000; it is capped at that amount.
• The remaining settlement money is divided evenly among class members with timely, valid claims.
• The fund is reduced first by approved loss claims, monitoring services, notice and administration costs, court-approved fees and service awards, and a category the notice calls Settlement Subclass Payments.

The notice describes online filing as the fastest option.

• Submit online by November 24, 2026.
• Or download a form and mail the completed, signed form to the Settlement Administrator.
• A mailed claim must be postmarked by November 24, 2026. Include supporting documents when claiming out-of-pocket losses.

The notice contains a case-number discrepancy in these instructions: elsewhere it identifies the case as 2:25-cv-00685-BJR, but the opt-out section uses 2:25-cv-00685-JBR.

• Opt out by mailing a written exclusion request postmarked by October 26, 2026.
• The request must include the proceeding name, your full name and current mailing address, your signature, and a clear statement that you want to be excluded.
• If you opt out, you receive no settlement benefits, but you keep any right you may have to sue separately about the issues in this case.

Objecting means asking the court not to approve some or all of the settlement. It does not remove you from the settlement, and you can still submit a claim.

• You may object only if you stay in the settlement.
• Mail your objection so it is postmarked by October 26, 2026.
• Your objection needs your contact information, specific reasons, supporting documents if any, information about any lawyer representing you, whether you plan to attend the hearing, certain information about prior class-settlement objections, and your signature.

Submitting a claim is the only way to seek benefits under this settlement.

• You will not receive settlement benefits.
• You will generally be bound by the settlement unless you opted out.
• You will give up the right to bring, continue, or join another lawsuit against LSC and the released parties over the claims resolved here.

The hearing date is not a payment date. The notice warns that an appeal could delay benefits.

• The final approval hearing is January 4, 2027, at 10:00 A.M. PT, by Zoom.
• Benefits can be distributed only if the court grants final approval and appeals are resolved or the time to appeal ends.
• The notice does not promise a specific payment or service-start date.
