ConnectOnCall Data Incident Settlement
This proposed settlement concerns a cybersecurity incident involving the ConnectOnCall Platform. Eligible U.S. residents may submit a claim for monitoring and one cash-payment option. The Court must approve the settlement before benefits are provided.

Who Is Included
Living U.S. residents whose information may have been impacted in the ConnectOnCall Platform Data Incident between February 16, 2024 and May 12, 2024, excluding defendants’ directors, officers, and employees; the assigned judges; their immediate family; and Court staff.
- Proof required: For Cash Payment A, provide a description of documented losses and supporting documentation that is not self-prepared, plus the required attestation. Cash Payment B requires an election on the Claim Form and an attestation under penalty of perjury, but no loss documentation. The notice states that reimbursed losses are not covered.

What monitoring service is offered?
A timely, valid Claim Form is required to receive monitoring. If you also qualify for cash, the notice presents monitoring together with either Cash Payment A or Cash Payment B.

- The service includes dark web monitoring, medical identity monitoring, real-time alerts, and up to $1 million in insurance coverage for medical identity theft.
- The notice estimates this two-year service at $360 per Settlement Class Member.
What is Cash Payment A, and what documentation does it require?
You cannot be paid again for the same loss if another source has already reimbursed it. If the administrator rejects a Payment A claim and you do not fix it, the claim will be changed to a Cash Payment B claim.
- Cash Payment A is for actual, documented, unreimbursed costs, expenses, losses, or charges tied to identity theft or fraud, a falsified tax return, or other possible misuse of private information attributed to the incident.
- It can be up to $5,000.
- You must choose Payment A on the Claim Form, attest under penalty of perjury that you had the specified after-hours communication with a healthcare provider or office, describe the losses, and submit supporting records that you did not prepare yourself.
What is Cash Payment B, and do I need loss documents?
Payment A and Payment B are alternatives. The notice uses “AND” between monitoring and the cash options, then “OR” between Payment A and Payment B.
- Cash Payment B is an alternate cash payment of up to $75 for people without documented losses.
- Choose Payment B on the Claim Form and attest under penalty of perjury that you communicated after-hours with a healthcare provider or office between May 12, 2014 and May 12, 2024.
- The notice does not require loss documents for this option.
Are the cash-payment amounts guaranteed?
The listed cash amounts are maximums, not guaranteed payment amounts.
- The settlement fund is $4,950,000, but it also pays administration costs and any court-awarded attorney fees, costs, and service awards.
- Cash payments can be reduced proportionally if valid claims use up the fund.
- If reductions are needed, the fund is distributed in this order: monitoring first, Cash Payment A second, and Cash Payment B third.
How do I submit a claim?
The notice does not describe the detailed contents of the online form.
- Submit a timely, valid Claim Form online by November 2, 2026, or mail it to the Settlement Administrator using the address on the Claim Form. Mailed forms must be postmarked by November 2, 2026.
- Choose the benefit option you want. Payment A requires the loss description and supporting records; both cash options require the stated attestation.
- Claim Forms are available through the settlement contact information in the notice, including by telephone or mail.
When will benefits be provided?
A hearing date does not mean the settlement has been finally approved or that benefits will be distributed then.
- Benefits are provided only after the Court approves the settlement and it becomes final.
- The notice does not give a payment or monitoring-start date.
- The Final Approval Hearing is scheduled for November 17, 2026, at 10:00 a.m., but the hearing date and time may change.
What happens if I opt out?
If you opt out and also submit a claim, the opt-out controls and you give up settlement benefits.
- Opting out means leaving the settlement. You will not receive monitoring or a cash payment, but you will not be bound by the settlement or its judgments.
- To opt out, mail a personally signed written request with your name, address, telephone number, email address if any, and a statement that you want to be excluded.
- The request must be postmarked by October 19, 2026. You cannot opt out by phone or email.
How is objecting different from opting out?
You may submit a claim if you object. You cannot object if you opt out.
- An objection lets a Settlement Class Member tell the Court they disagree with some or all of the settlement while remaining in the class.
- You must file a written objection with the Court and send copies to Class Counsel, Defendants’ Counsel, and the Settlement Administrator by October 19, 2026.
- An objection must include specific information, including your contact information, reasons for objecting, information about any lawyer representing you, whether you or your lawyer will appear at the hearing, and your signature.
