What information will I need for a claim?

• A claim must be completed and submitted under penalty of perjury. The notice says it generally asks which products you bought, how many you bought, and approximate purchase dates.
• The notice does not give a fixed list of documents to attach, such as receipts. The Settlement Administrator can ask for more information to check a claim, and incomplete or inaccurate information may lead to denial.

• The representatives may ask for up to $2,500 each as service awards, but the court decides whether to approve those amounts.
