Fairchild Medical Center Pixel Settlement – Expected $25 Cash Payment

The benefits are not alternatives. The notice says Class Members may use the privacy service and also seek the cash payment.

• You may enroll in one year of CyEx Privacy Shield Pro.
• You may also submit a claim for the one-time cash payment expected to be $25.

CyEx Privacy Shield Pro is a one-year privacy service. It includes dark-web and compromised-password scans, a virtual private network (VPN), a password manager, other privacy tools, and access to a fraud-resolution agent if suspicious activity occurs.

• Enrollment codes were sent to Class Members by postcard.
• If you no longer have your code, contact the Settlement Administrator for help by phone, email, or mail.
• The notice does not give the detailed enrollment steps or say whether a Claim Form is used for this service.

A Claim Form is required to request the cash payment. The notice describes online and mail submission options.

• Submit a Claim Form online by November 2, 2026.
• Or download a printable form, complete and sign it, and mail it so it is postmarked by November 2, 2026.
• You can request a paper Claim Form from the Settlement Administrator by phone, email, or mail.

The notice does not require documents to support a cash-payment claim.

• No proof or explanation is required for the cash-payment claim.
• The payment is expected to be $25, not guaranteed to be that amount.

The final approval hearing is scheduled for October 15, 2026. That hearing date is not a payment date, and the notice does not provide a specific distribution date.

• The Court must grant final approval.
• Any appeals must be resolved before settlement payments are distributed.

To leave the Settlement, you must send a valid Request for Exclusion by September 28, 2026. If you opt out, you cannot receive Settlement benefits, but you keep any rights you may have to bring your own case about these issues.

• If you stay in the Settlement and take no action, you will not receive a benefit.
• You will give up the rights described in the Settlement Agreement's release section, including the ability to be part of another lawsuit against Fairchild Medical Center over the issues the Settlement covers.

Objecting means asking the Court not to approve some or all of the Settlement. It is different from opting out. The notice says a person whose objection is overruled will have another opportunity to opt out.

• Only Class Members who do not opt out may object.
• File the complete objection with the Clerk of Court by September 28, 2026.
• Also send copies to the Settlement Administrator, Class Counsel, and Fairchild Medical Center's counsel.
• An objection must include information showing that you are a Class Member, such as a notice you received, along with the other required information.
