What do I need to show that I am part of the settlement?

What types of out-of-pocket expenses may be covered?

- Send a signed letter saying that you want to be excluded from the settlement in Bueno v. Etairos Health, Inc., Case No. 24-002129-CI.
- Include your name and address.
- Mail it to the Claims Administrator with a postmark no later than November 20, 2026.
- If you exclude yourself, you cannot receive settlement benefits, but you are not bound by the judgment.
