Boone Health Patient Portal Settlement

The settlement covers a specific group of Missouri Boone Health patients. The notice does not decide whether any particular person qualifies.

• You must be or have been a Boone Health patient.
• You must live in Missouri.
• You must have logged into the Boone Health Patient Portal between April 1, 2021 and December 5, 2022.
• The settlement excludes the judge evaluating the settlement, court personnel and their immediate families, and Boone Health and the related people and entities listed in the notice.

The code for Privacy Shield Pro is provided automatically, but enrollment still requires using the code. The notice says separate enrollment instructions will be sent by email.

• Privacy Shield Pro: Every Settlement Class Member will receive a code to enroll. The code is at the top of the member's notice.
• Cash payment: A class member who submits a timely, valid Claim Form and chooses the cash option may receive $20.
• These are not alternatives. The notice says Privacy Shield can be added to a cash claim.

The notice identifies the form and credentials needed to make a cash claim. It does not state whether any additional supporting documents are required.

• Complete and submit a Claim Form online or by mail.
• Provide the Settlement Class Member Login ID and Password from the notice you received by email or mail.
• If you did not receive those credentials, email [email protected] to verify your identity and obtain them.

A Claim Form is required for the cash payment. The notice gives the time zone only for online claims.

• Submit an online Claim Form by 11:59 p.m. PST on November 9, 2026.
• Or mail a paper Claim Form postmarked by November 9, 2026.

The hearing date is not a payment date. Payment depends on final approval, the end of any appeals, and approval of the individual claim.

• The Court's final approval hearing is scheduled for December 1, 2026, at 8:30 a.m.
• If the Court approves the settlement, approved claimants will be paid 60 days after final approval and after any appeals are complete.
• Payment will be by check unless the claimant chooses PayPal, Venmo, or Zelle.
• Checks expire 60 days after issuance.

Excluding yourself means leaving the settlement. You will receive no settlement benefits, but you keep any rights you currently have to sue Boone Health about the claims in this case.

• Send a written exclusion request by mail or delivery no later than October 9, 2026.
• Include the case name and number, your name, current address, telephone number, unique ID, signature, and a statement requesting exclusion.
• Send it to Boone Health Settlement, c/o Atticus Administration, PO Box 64053, St. Paul, MN 55164.

Objecting asks the Court not to approve some or all of the settlement. It is different from excluding yourself: a person who excludes themselves cannot object.

• You must be a Settlement Class Member and remain in the settlement to object.
• File a personally signed letter or brief with the Court, and mail copies to Class Counsel and Defendants' Counsel.
• The objection must include the information and statements listed in Question 17 of the notice.
• It must be filed and mailed by November 9, 2026.

The Court may change the hearing date or time without further notice.

• The hearing is scheduled for December 1, 2026, at 8:30 a.m., at the Cole County Courthouse, Division Three, 301 East High Street, Jefferson City, Missouri 65101.
• The hearing may be held remotely; the notice says remote instructions would be posted on the Settlement Website.
• Attendance is optional. A timely written objection can be considered even if the objector does not attend.
