Safe Fleet Data Incident Settlement

Can I get credit monitoring and a cash payment?
The credit-monitoring benefit can be claimed along with one cash-payment option.
- You can receive two years of CyEx Financial Shield Complete and choose one cash option.
- Your cash choice is either up to $2,500 for qualifying documented losses or the $50 alternative payment. You cannot receive both cash payments.
What losses may qualify for the up-to-$2,500 payment?
The documented-loss option may reimburse actual out-of-pocket losses of up to $2,500 that resulted from the data incident, or were more likely than not caused by it. The loss must have occurred between April 13, 2024, and December 2, 2026. You also must have made reasonable efforts to prevent the loss or obtain reimbursement.
- Identity theft or fraud losses
- Costs for credit reports, credit monitoring, or credit freezes and unfreezes
- Replacing identification documents
- Postage used to contact banks by mail
What documentation do I need for a documented-loss claim, and what is excluded?
For the documented-loss option, a personal statement by itself is not enough. The notice also excludes payment for emotional distress, personal or bodily injury, and punitive damages. In contrast, the $50 alternative payment requires no proof or explanation.
- Provide third-party records, such as bank statements or receipts, showing the amount you spent or lost.
- You may include your own notes, declarations, or affidavits to add context, but they cannot be the only proof.
- Do not claim expenses already repaid by another source.
What does the two-year credit-monitoring benefit include?
The notice says the CyEx subscription becomes active after the Court grants final approval of the settlement.
- The service includes up to $1 million in financial fraud insurance.
- It monitors fraud or identity theft, unauthorized financial transactions, and personal information linked to high-risk transactions.
- A fraud-resolution agent is available if suspicious activity occurs.
How do I submit a claim, and what is the deadline?
You can submit online or download and mail a form. You may also ask the Settlement Administrator for a paper form by phone, email, or mail.

- Or mail a completed, signed Claim Form; it must be postmarked by December 2, 2026.
- Include supporting documentation with a mailed claim when documentation is required, such as for a documented-loss request.
When would settlement payments be sent?
The notice does not give a specific payment date. It says valid claims filed by the deadline will be paid after final approval and after any appeals have been resolved.
- Benefits cannot be distributed unless the Court approves the settlement and it becomes final.
- Payments also cannot be distributed while appeals are pending.
- Keep the Settlement Administrator informed if your contact information changes after you submit a claim.
What do I need to do if I want to opt out?
Opting out means leaving the settlement. You would receive no settlement benefit or payment, but would keep any rights you may have to bring your own lawsuit about the issues in this case.
- Send a written Request for Exclusion to the Settlement Administrator.
- Include the case name and number, your full name, mailing address, telephone number, email address if any, your signature, and a clear statement that you want to be excluded.
- The request must be submitted, postmarked, or emailed by November 2, 2026.
