Benefit Summary

Eligible people who received customized Claim Forms may seek reimbursement of listed $15 medical copays. Proof of payment can support up to full reimbursement. Without proof, a signed affirmation can support up to 40% reimbursement, capped at $100 total per person. Payments may be reduced pro rata if approved claims exceed the net settlement amount.

Who Is Included

The settlement class includes Medicare-eligible retired New York City employees and Medicare-eligible dependents enrolled in the EmblemHealth-administered medical GHI Senior Care Plan at any time from January 1, 2022 through January 31, 2023. Only members whom Emblem records identify as potentially exposed to eligible copays receive customized Claim Forms and may seek reimbursement.

• Proof required: A completed signed customized Claim Form is required. Proof of each copay is required for up to full reimbursement; without proof, the claimant must affirm payment and can receive only partial reimbursement. Name-change or representative-authority documents are required when applicable.
