CVS Digital Privacy Settlement

You may be part of the settlement if you meet the class definition. The notice does not require that you made a purchase or that you have a particular type of data-loss claim.

• All living people who accessed CVS Digital Properties while in the United States before July 27, 2026 are included, unless they properly exclude themselves.
• The notice describes CVS Digital Properties as including CVS websites and the CVS app, but does not provide a complete list in the notice itself.

The actual cash payment may be lower because payments can be reduced in proportion to the number of valid claims. You may also owe any applicable taxes on a payment.

• You may submit a valid claim without documents for up to $5.
• You may submit a valid claim with valid proof that you are a Settlement Class Member for up to $10.
• These are alternatives: the notice describes payment of either $5 without documentation or $10 with proof, not both payments together.

No documentation is required for the up-to-$5 option. A signature, declaration, or notice ID is not identified in this notice as proof for the $10 option.

• The notice does not identify which documents count as valid proof of Settlement Class membership.
• For the $10 option, the Claim Form or the Settlement Agreement may provide further detail, but this notice alone does not list acceptable proof.

A claim is required for either cash-payment option. Doing nothing will not result in a payment.

• Submit a valid Claim Form online through the settlement website, or download the form and mail it to the Settlement Administrator.
• The claim deadline is 11:59 p.m. EST on November 16, 2026.
• The mailing address is CVS Digital Privacy Settlement, c/o Settlement Administrator, PO Box 1110, Costa Mesa, CA 92628.

The December 1, 2026 hearing is not a payment date. Approval and the end of any appeals are still required before benefits are distributed.

• The Court must first approve the settlement, and any appeals must be resolved.
• The notice says qualifying claimants will receive payment 120 days after final approval and/or after the appeals process is complete.
• Payments may be sent by PayPal, Venmo, Zelle, or check, based on the form of payment elected. A check expires 180 days after it is issued.

Excluding yourself, also called opting out, means leaving the settlement. You cannot receive a settlement payment if you opt out.

• If you stay in the settlement, you give up the right to sue the defendants and other released parties over the claims resolved by this settlement.
• This release applies even if you do not file a claim.
• To preserve any rights you may have to sue on your own about these issues, you must formally exclude yourself instead.

Send a mailed request to CVS Digital Privacy Settlement, ATTN: Exclusion Requests, PO Box 1110, Costa Mesa, CA 92628.

• Send a written exclusion request by 11:59 p.m. EST on November 1, 2026.
• It must state that you want to be excluded from this case’s settlement; include your name, address, telephone number, email address if any, a statement that you accessed CVS Digital Properties before July 27, 2026, and your signature.
• An exclusion request must be personally signed. A mailed request must be postmarked no later than November 1, 2026.

The notice says the objection must meet every listed requirement to be valid. It also requires additional information about lawyers assisting with an objection and certain past objections.

• You may object only if you remain in the settlement; people who opt out cannot object.
• File a written objection with the Court and mail copies to Class Counsel and defendants’ counsel by November 1, 2026.
• The objection has detailed content requirements, including your contact information, the basis for claiming class membership, grounds and supporting materials, and your personal signature.

At the hearing, the Court will consider whether to approve the settlement and requests for attorneys’ fees, costs, and service awards.

• The Final Approval Hearing is scheduled for 9:30 a.m. EST on December 1, 2026, by Zoom.
• You do not have to attend.
• To ask to speak, you must timely object and include a Notice of Intent to Appear with the required information. The hearing date or time may change.
